Hales Homecare is seeking compassionate and dedicated
Hospital Discharge Support Workers to join its team in Gateshead. This role involves helping individuals transition from hospital care back to living independently in their own homes.

  (covering NE10, NE9, NE8, DH3, NE16, NE11, NE40, NE39, NE17, NE21)
• Pay: £12.30 per hour
• Contract Type:
  Full-Time or Part-Time
• Licence Requirement:
  Full UK driving licence and access to own vehicle (essential)
• Experience Requirement:
  Previous care experience (essential)
• Eligibility:
  Must have the right to work in the UK

Support Workers will provide hands‑on care tailored to individual needs, ensuring continuity of care and supporting long‑term recovery. The role requires strong communication skills, reliability, and a commitment to delivering high‑quality, personalised care.

• Provide hands‑on care and support tailored to individual needs
