Job Summary

Gateshead Health require qualified a District Nurse todeliver high standards of clinical practice that hold patients and their families at the centre to ensure that they receive safe, quality care that gives them a good experience. To identify and assess the core needs of patients and carers, being responsible for delivery of an identified package of care.,

• To assume 24-hour responsibility for a designated case-load.
• To maintain standards of care through evaluating and implementing evidence-based practice.
• To lead and develop staff and the service provided.
• To delegate work appropriately according to skill mix ensuring maintenance of safety within the community.
• To support the locality Team Leader in achieving their objectives.

Based in the North East of England we provide a range of hospital and community health services from our leading facilities, including the Queen Elizabeth Hospital, Blaydon urgent care centre and Bensham Hospital, all within Gateshead. Established in 2005, we were one of the first foundation trusts in the country and since then have consistently achieved the highest levels of care for patients.
