Home Group in Blaydon/Winlaton is seeking a Support Coordinator to lead planning and coordinate Support Workers delivering person-centred support for customers with learning disabilities in Gateshead area.
You will balance safeguarding, admin and meetings, manage caseloads, and help customers with budgeting, cooking and appointments. A Level 3 Diploma in Care or equivalent is preferred, with experience in referrals and coordinating support.
