WORK SCHEDULE:
Monday-Friday, 40 hours/week
FLSA CLASSIFICATION: Full-Time, Exempt
COMPENSATION: The hiring range for this position is $110,000–$125,000, with the opportunity to earn additional compensation through bonuses; candidates may also choose between a base salary structure or a variable compensation plan (commission), which may provide higher earning potential

Position Summary
Vitae Health is seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered medical care to residents in a skilled nursing facility. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life.
Key Responsibilities
Perform comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized, evidence-based care plans for residents in a skilled nursing setting
Provide ongoing medical management including ordering and interpreting diagnostics, prescribing treatments, conducting medication reviews, and proactively monitoring conditions to prevent complications and reduce hospitalizations
Deliver timely clinical interventions in response to changes in patient status, ensuring high-quality, patient-centered care that improves outcomes and supports functional well‑being
Support value-based care initiatives by focusing on quality outcomes, reducing avoidable hospital readmissions, optimizing cost of care, and utilizing data to guide clinical decision-making and performance improvement
Collaborate closely with physicians and interdisciplinary team members to ensure coordinated, efficient care, and participate in care plan meetings and quality improvement efforts
Facilitate smooth transitions of care across settings, ensuring continuity, reducing gaps in treatment, and supporting successful admissions and discharges
Educate residents and families on diagnoses, treatment plans, medications, and preventative care strategies to promote engagement and long‑term health
Maintain accurate, timely, and compliant documentation while adhering to all clinical, regulatory, and organizational standards
Qualifications
Master’s degree in Nursing from an accredited Nurse Practitioner program
Active and unrestricted Advanced Practice Nurse (APN) license in respective state
Current DEA registration and prescriptive authority (or eligibility)
Board certification as a Nurse Practitioner (e.g., FNP, AGNP)
2 years of clinical experience preferred, ideally in geriatrics, long-term care, or post‑acute settings
Strong clinical assessment, diagnostic, and treatment planning skills
Knowledge of chronic disease management and geriatric care principles
Ability to deliver high-quality, value-based care focused on outcomes, cost‑efficiency, and reduced hospitalizations
Proficiency in evidence-based practice and clinical decision-making
Ability to work both independently and collaboratively within an interdisciplinary team
Excellent communication and interpersonal skills with residents, families, and staff
Strong organizational, time management, and documentation skills
Familiarity with electronic health records (EHR) systems
