Indigenous Transitions Facilitator at RVH

As an Indigenous Transitions Facilitator, you will coordinate discharge processes and optimize care planning for Indigenous patients. You will utilize your clinical experience and understanding of Indigenous cultural practices to liaise with families, navigate home care services, and ensure access to community supports.
Your role is vital to providing equitable, culturally safe care.
Key Responsibilities:
• Develop individualized care planning for patients
• Serve as primary point of contact for Indigenous patients
• Educate families on community supports and resources
• Conduct medication reconciliation and health teaching
• Collaborate with interdisciplinary teams for discharge coordination
Requirements:
• Registered health professional with current certification
• Bachelor’s degree in Nursing or related field
• Minimum 5 years of clinical experience
• Knowledge of community resources and systems
• Commitment to providing culturally safe care
