Position: Social Worker for Rehab Programs
Join UHN Reactivation Care Centre as a full-time Social Worker in the Transitional Care Program, specializing in patient-centered care and rehabilitation support. Collaborate with teams to enhance patient well-being.

As a Social Worker, you’ll play a vital role in assessing patients and developing care plans that promote independence and quality of life. The position involves working alongside a multidisciplinary team, addressing social determinants of health, and managing transitions to home care or other settings. Strong communication and problem-solving skills are essential for this impactful role.

Key Responsibilities:

• Assess patient psychosocial needs and formulate treatment plans

• Liaise with community organizations and healthcare teams

• Provide clinical teaching to students and peers

• Develop inpatient education initiatives

• Engage in research and program planning

Requirements:

• M.S.W. or recognized equivalent

• OCSWSSW membership

• Experience in health-care settings

• Strong communication and time-management abilities

• Familiarity with social welfare legislation
