About the Opportunity Are you a dedicated Family Nurse Practitioner looking for a rewarding primary care role with excellent work-life balance? A respected regional healthcare organization is seeking a full-time Family Nurse Practitioner (FNP) to join an expanding outpatient primary care team in New York.

This outpatient-only position allows you to build meaningful, long-term relationships with patients across all stages of life. You will practice in a collaborative, community-focused clinical environment supported by the resources, technology, and specialist network of a major integrated healthcare system.

Key Responsibilities

• Comprehensive Primary Care: Provide full-scope outpatient care, including routine wellness visits, acute care diagnosis, and chronic disease management across the lifespan.
• Patient Centered Care: Form long-term relationships with patients and families through high-touch continuity of care.
• Interdisciplinary Collaboration: Work closely alongside physicians, advanced practice providers (APPs), and dedicated clinical support staff.
• Clinical Excellence: Maintain accurate clinical documentation within modern EMR systems and participate in evidence-based quality improvement initiatives.

Qualifications

Requirements:

• Active (or eligible for) Family Nurse Practitioner (FNP) license in the State of New York.
• Active (or eligible for) Registered Nurse (RN) license in the State of New York.
• Board Certification as a Family Nurse Practitioner (AANP or ANCC).
• Excellent communication, diagnostic, and interpersonal skills.

Preferences:

• 1+ years of experience in primary care, family medicine, internal medicine, or community health (New grads with strong clinical rotations are welcome to apply).
• Comfort with electronic medical records (EMR) systems.
