LPN – Nursing

Full-time nights, every other weekend and holiday.

1 year experience as an LPN required.

• Current NY LPN Licensure
• BLS certification (AHA or RQI only)

• Provide direct and indirect skilled nursing care for assigned residents.
• Collect data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team.
• Contribute to the creation of individualized care plans, reflecting nursing diagnoses and interventions.
• Recommend revisions to care plans based on resident evaluations and outcomes.
• Collaborate with members of the healthcare team to ensure comprehensive care delivery.
• Participate in mentoring and orientation of staff, maintaining knowledge and competency in nursing practice.
