• Current NY LPN Licensure
• BLS certification (AHA or RQI specifically)
• 1 year of experience as an LPN

• Collect and communicate data relevant to resident's physical and psychosocial condition to the RN and interdisciplinary team.
• Contribute to creating a developmentally appropriate and culturally relevant care plan involving residents, families, and healthcare teams.
• Recommend revisions to the care plan based on resident outcomes and collaborate with the healthcare team to ensure comprehensive care.
• Maintain knowledge and competency in nursing practice and participate in the orientation and mentoring of staff.
• Deliver care considering safety, cost, and impact, providing leadership under the direction of the RN.
