Community Support Facilitator

About the Role

Our client is looking for an empathetic and resourceful Community Support Facilitator to join their team in George . This role is dedicated to empowering individuals and families within the community by facilitating access to essential support services and promoting social inclusion. You will work directly with clients to understand their unique needs, guide them through available resources, and provide practical assistance to improve their overall well-being.

The ideal candidate is passionate about community development and possesses excellent interpersonal skills to build trust and rapport with diverse populations in the Western Cape region.

• Assist clients in identifying their needs and navigating the complexities of social support systems.
• Provide practical guidance and support for accessing services such as financial aid, healthcare, and educational opportunities.
• Develop and maintain strong relationships with local community organizations and service providers.
• Organize and run support groups and community information sessions.
• Monitor client progress and provide follow-up support to ensure positive outcomes.
• Contribute to program development and evaluation based on community feedback and needs.

• A relevant qualification in Community Development, Social Work, or a related field is advantageous.
• Previous experience in a community-facing role, assisting vulnerable populations.
• Demonstrated knowledge of local support services and resources within the George area.
• Excellent communication and active listening skills.
• Ability to work independently and collaboratively as part of a team.
• Strong organizational skills and attention to detail in client record-keeping.
