About the Role
Our client is seeking a dedicated Disability Support Officer to join their compassionate team in George . This role is crucial in empowering individuals with disabilities to live fulfilling and independent lives within their community. You will work closely with clients to understand their unique needs, develop personalized support plans, and facilitate access to services and opportunities. Key Responsibilities- Assess client needs and develop tailored support strategies and care plans in consultation with individuals and their families.
- Provide practical assistance with daily living activities, including personal care, household tasks, and community participation.
- Facilitate clients' engagement in recreational, educational, and vocational activities to enhance their skills and social connections.
- Advocate for clients' rights and needs, ensuring they have access to appropriate services and support networks within the Western Cape.
- Collaborate with healthcare professionals, government agencies, and community organizations to ensure coordinated and comprehensive support.
- Maintain detailed records of client interactions, progress, and any required reporting, adhering to strict confidentiality standards.
- Relevant qualification in Social Work, Psychology, Community Development, or a related field.
- Minimum of 3 years of experience in disability support or case management.
- In-depth knowledge of disability services, NDIS principles (if applicable), and support frameworks.
- Strong empathy, patience, and excellent interpersonal and communication skills.
- Ability to work flexibly, including occasional evening or weekend support as needed in George.
- Proficiency in using relevant software for case management and record-keeping.
