The Patient Assessment Coordinator coordinates the process of providing accurate and timely data to the appropriate entities involved in the Prospective Payment System and quality reporting program for Medicare patients.
Responsibilities include:
- Oversees all aspects of data collection on the IRF/PAI.
- Ensures presence of supporting documentation.
- Provides staff education to ensure compliance with CMS guidelines.
- Reviews each IRF/PAI form completely, accurately, and timely.
- Monitors functional status scoring accuracy and reflects patient care needs.
- Inputs appropriate information into the eRehab
Data system. - Coordinates collection of patient information from the medical record.
- Sets completion dates for the multidisciplinary team.
- Develops and interprets policies and procedures.
- Serves as primary resource for problem‑solving regarding Medicare IRF prospective payment system (FIM scoring, impairment group coding, tiering distribution).
- Handles coverage and classification criteria and quality reporting requirements.
Qualifications:
- Graduate of an accredited OT, PT, SLP, or RN program with current licensure in the state of the facility. ASN required;
Bachelor’s preferred for nursing. Master’s required for OT/PT/SLP. - Current Basic Life Support (BLS)/CPR card.
- Minimum 3–5 years experience as a patient assessment coordinator may substitute for degree and license.
- Minimum 3–5 years experience caring for patients in acute hospital, rehabilitation, or LTACH setting.
