This is an opportunity to take your nursing skills beyond a traditional healthcare setting and provide comprehensive, community-based care to individuals living with severe and persistent mental illness and co-occurring conditions.

As an RN Staff Nurse, you’ll provide medication management, clinical and psychiatric assessments, physical health support, health education, and care coordination while working alongside an interdisciplinary behavioral health team committed to recovery and independence.

About Delaware Intensive Case Management

The Delaware Intensive Case Management program of Resources for Human Development (RHD) provides community-based treatment and support to individuals living with severe and persistent mental illness and co-occurring conditions.

Our team takes an individualized, recovery-focused approach to care, helping participants address behavioral and physical health needs while developing the skills and supports needed to achieve greater stability and independent functioning in the community.

As the RN Staff Nurse, you’ll serve as an important connection between participants, medical providers, behavioral health professionals, families, and community resources to help ensure each person receives comprehensive and coordinated care.

What You’ll Do

As the RN Staff Nurse
, you will provide comprehensive nursing care and clinical support to individuals living with severe and persistent mental illness and co-occurring conditions.

You’ll manage medications and treatments, conduct clinical and psychiatric assessments, address physical health needs, educate participants and families, and coordinate care with medical providers and the interdisciplinary treatment team.

Your Responsibilities May Include:

Nursing Assessment & Clinical Care

  • Conduct psychiatric, clinical, and physical health assessments.
  • Provide nursing care and monitor participants' ongoing health status.
  • Conduct vital signs, metabolic screenings, injections, and other ordered treatments.
