Full‑time Reentry Case Manager – Sussex County, DE
Pay range: $42,000.00/hr – $45,000.00/hr

Justice Works is seeing a Reentry Case Manager who will play a vital role in laying the foundation for a successful transition back into the community. In this role, you will engage Youth from the moment they enter placement, conducting orientation sessions to introduce them to program expectations, available resources, and the importance of family involvement. You’ll build trust using a strength‑based approach, fostering motivation and positive relationships that are critical for successful reintegration.

Responsibilities- Facilitate orientation sessions for Youth entering placement, outlining program expectations and available supports.
- Initiate contact with families/caregivers to introduce them to the reentry program and clarify expectations.
- Build meaningful connections with Youth and families to support a smooth transition back into the community.
- Collaborate with Youth to develop individualized reentry plans focused on their strengths, goals, and needs.
- Provide structured engagement, comprehensive planning, and dedicated aftercare support to promote long‑term success.
- Work to reduce recidivism by equipping Youth with the tools, resources, and support systems they need to lead productive, law‑abiding lives.
- Transport families using personal vehicle as necessary.
- Bachelor’s Degree required.
- Valid driver’s license, auto insurance and access to a vehicle.
- Pass current, applicable clearances.
- Pass pre‑employment, post‑offer drug screening.
- Ability to work non‑traditional hours including evenings and weekends.
