A Brief OverviewUnder general supervision, the Staff Pharmacist oversees the supervisory, distributive, and clinical functions of the Pharmacy including the prompt and accurate reviewing, processing, checking, and monitoring of physician orders, patient medication profiles, and completed prescriptions. Accurate drug information is promptly provided to health care professionals and patients. Assists the Director and Assistant Director of Pharmacy in their duties to maximize the performance of the department and to implement and improve services as the needs of the department, the hospital, and the community change.
Duties delegated by the Director or the Assistant Director of Pharmacy Services are accurately and promptly completed. The Staff Pharmacist maintains an appropriate level of competency by completing at least 15 Continuing Education (CE) credit hours to include 6 CE live credit hours per one (1) licensing year. Performs other duties as assigned.
- Follow policy and procedures on all technical aspects of the job to include timeliness and accuracy of order entry which includes reviewing daily patient profiles for allergies, weights, age, while monitoring for appropriateness of patient, dose, route, frequency and time.
- Check alerts /warnings such as duplicate therapy, interactions and then document a reason for overrides or follow up.
- Complete and document medication reconciliation according to policy.
- Check all meds pulled by Techs, documenting any near misses. Alerts are routinely addressed and if an override occurs, are documented in the EPIC.
- Participate in Clinical Pharmacy Services to include reviewing daily patient profiles, appropriate labs and monitoring orders for renal/hepatic dosing, indications on prn meds, max and min doses.
- Monitor for IV to PO switches.
- Perform drug therapy consults such as aminoglycoside, vancomycin & TPN dosing.
- Provide current & accurate drug information to all customers.
- Evaluate patient profiles for compliance with core measure standards.
- Doctoral Degree from a college of pharmacy required.
- Previous experience in hospital pharmacy preferred.
- Ability to work under direct or minimal supervision and handle stress appropriately required.
- Ability to work cooperatively with health‑system and pharmacy staffs required.
- Ability to handle frequent interruptions and adapt to changes in workload and work schedule required.
- Ability to set priorities, make decisions, and respond quickly to emergency requests required.
- Communication, supervisory, organizational, learning, and minimal teaching skills are required.
- Registered Pharmacist - South Carolina Department of Labor, Licensing and Regulation by the State of South Carolina Board of Pharmacy required.
