Per-Visit Rates:
- $75 per regular OT Visit
- $112.50 per OT Eval
- $187.50 per OT SOC
Mileage reimbursed at $0.50 per mile.

Job Description: Occupational Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.
- Evaluate patients and perform appropriate therapy interventions
- Develop a comprehensive plan of care and coordinate multidisciplinary care
- Perform prompt clinical documentation and visit notes
- Continue communication with physician, care team members, patient, and family
- Comfortable working in all aspects of the home health setting
- Manage patient treatment and outcomes directly, or through COTA supervision
Requirements:
- Strong organization skills and time management
- Able to multitask and accomplish and meet timelines
- Professional appearance and effective communication skills
- Current Texas Occupational Therapist (OT) license
- One year of occupational therapy experience
- Current Texas driver's license
- CPR certification
- Experience with healthcare software (Homecare Homebase preferred)
- Previous Home Health experience is helpful, but not required
