Join Adaptive Home Health as an RN Case Manager and deliver high-acuity, patient-centered care throughout Texas. Your compensation will depend on your experience and skill set.

Seeking elite Registered Nurses with wound care and infusion experience. Previous home‑health experience is highly preferred. The role is full‑time with an expectation of 30 productivity points per week. Clinicians are paid per point and can earn additional pay for exceeding the baseline.

• Evaluate patients and home health environments; complete skilled home health assessments, documentation, and visit notes.
• Develop care plans and communicate with physicians, care team members, patients, and families.
• Comfortable treating wounds and performing infusions.
• Advocate for patients; maintain ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care.
• Administer medications and treatments as prescribed; provide direct patient care including infusion therapy, wound care, teaching, and disease management.
• Supervise nursing clinical care team members (LVN’s & HHA’s).
• Participate in regular case conferences and in‑services.

• Strong organization skills and time management.
• Able to multitask and meet timelines.
• Professional appearance and effective communication skills.
• Current Texas Registered Nurse (RN) license.
• Three years of clinical nursing experience.
• Current Texas driver’s license.
• CPR certification required.
• Experience with healthcare software (Homecare Homebase preferred).
• Competency to carry out any skilled procedure within the scope of the Nurse Practice Act.
