Altus Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.
Schedule:
Monday - Friday, 8 am - 4:30 pm, On-call rotation required.
Territory:
Williamson and surrounding counties. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey as part of our interdisciplinary care team.
