Physician Assistant (Effingham Care & Rehabilitation Center)

Strategis Consulting is hiring a Physician Assistant for the Effingham Care & Rehabilitation Center. This role involves providing direct patient care to residents under the supervision of the lead physician.

Provides comprehensive medical services with a focus on geriatrics, chronic disease management, and rehabilitation. Services include preventive care, immunizations, primary care and treatment, health education, medication prescribing, laboratory ordering and interpretation, physical examinations, and nutrition counseling. The primary objective is to deliver high‑quality, holistic in‑place care to minimize unnecessary hospitalizations while ensuring compliance with The Joint Commission standards, federal, state, and local regulations, and internal policies.

• Provide direct patient care to residents under physician supervision.
• Conduct comprehensive medical histories, physical examinations, and functional status assessments.
• Formulate tentative diagnoses, order appropriate laboratory tests, and transmit findings to the supervising physician.
• Develop, implement, and manage individualized treatment plans in collaboration with the multidisciplinary team.
• Perform therapeutic procedures such as injections, immunizations, suturing, wound management, and infection control.
• Order, interpret, and evaluate diagnostic tests, including lab work and imaging studies.
• Prescribe medications and therapeutic interventions as authorized by state law and the supervising physician.
• Monitor patient progress, adjust care plans, and document outcomes in the electronic health record.
• Educate residents and families about illnesses, treatments, preventive care, and health maintenance.
• Collaborate with the healthcare team, facilitate referrals, and coordinate care across providers.
• Maintain accurate, timely, and confidential medical records in compliance with legal and professional requirements.
• Conduct daily or routine rounds to manage clinical care and address acute or emergent issues.
• Contribute to leadership initiatives and uphold a safe, clean working environment.

• Education:
  
  Bachelor’s or Master’s degree in Physician Assistant Studies, Health Science, Medical Science, or a related field.
• Certification:
  Current Physician Assistant certification, valid Georgia medical license, active DEA certificate (or eligibility), and NCCPA board certification.
• Licensure:
  Unrestricted license to practice medicine in Georgia.
• Certifications:
  
  BLS and ACLS required.
• Experience:
  
  Minimum two years as a PA in a nursing home or similar healthcare setting; experience in geriatrics, internal medicine, family medicine, or skilled nursing facilities preferred.

  Proficient in Microsoft Word, Excel, PowerPoint, and Outlook.

  Strong clinical judgment, excellent communication, compassionate care for the geriatric population, and ability to work independently and as part of a team.
