HOME HEALTH PHYSICAL THERAPIST

Job Title:
Physical Therapist – Home Health

Rate:
Up to $2200.00 weekly

Location:
Gray, GA

Day (8H)

• Required a degree in physical therapy (Bachelor’s or Master’s)

• Previous experience in home health care of similar setting is preferred.
• Must have recent Home Health experience.
• 1 full year Adult Home Health Experience.
• Must have reliable transportation.
• Must have current driver's license.

• Must hold a valid state license to practice physical therapy
• AHA BLS certification required.
• Must be willing to travel up to 75 miles from home clinic.

• Patient Assessment
  :
  Evaluate patients' physical function and limitations within their home environment to develop individualized treatment plans.
• Treatment Implementation
  :
  Provide therapeutic interventions aimed at improving mobility, strength, balance, and pain management.
• Education
  :
  Educate patients and caregivers on diagnoses, safety precautions, equipment use, and home exercise programs.
• Collaboration
  :
  Work with an interdisciplinary team, including nurses and occupational therapists, to create comprehensive care plans that address the patient's physical, emotional, and social needs.
• Documentation
  :
  Maintain accurate records of patient evaluations, treatment plans, and progress according to healthcare standards.
