Schedule: Full Time, 8 HR Day Shift
Position SummaryWorks as part an interdisciplinary team to facilitate the delivery of services to patients/families. Coordinates the discharge planning process. Uses assessment skills to accurately identify patients’ needs and demonstrates the ability to provide safe, therapeutic environment for all ages, pediatric, youth, adolescent, adult, and geriatric patients. Participates in patient/family teaching. Uses teaching literature and techniques for different age groups and learning needs.
Knowledge,Skills & Abilities
Bachelor’s Degree in Social Work/BA in Sociology with minimum of two years experience in social work. Previous experience as a Social Worker in a hospital setting strongly desired. Computer skills preferred. Ability to communicate clearly with a diverse population of patients/family units of all ages, variety of cultures, etc. Good organization, prioritization, and problem solving skills. Flexible in work schedule, assignments and work load.
Strong ability to establish and maintain effective working relationships with a variety of personnel at all levels within and external to the organization. Ability to interpret and implement the policies and procedures of South Georgia Medical Center, regulatory agencies’ standards and rules, reimbursement rules, etc.
