Home Health Physical Therapist Pay Rate: $60.68/hr

Home Health Physical Therapist Shift: Monday–Friday, 8:00 AM – 5:00 PM

Home Health Physical Therapist Mileage Reimbursement: $0.49/mile

Home Health Physical Therapist Licenses: GA Physical Therapist license required (compact licenses accepted)

Home Health Physical Therapist Preferred Skills/Experience:

• Home Health experience
• Homecare Homebase (HCHB) experience

Home Health Physical Therapist Job Description:
We are seeking a Physical Therapist (PT) to provide home health services across Spalding, Pike, and Henry counties in Georgia. This role involves evaluating and treating patients in their homes, developing individualized treatment plans, and helping patients achieve maximum independence and function.

Home Health Physical Therapist Responsibilities:

• Conduct comprehensive evaluations using appropriate tools and procedures
• Review patient records for diagnosis, medical history, and contraindications
• Establish measurable goals and develop treatment plans
• Regularly re-assess treatment effectiveness and modify plans as needed
• Communicate patient progress to the care team, physicians, and family members
• Recommend and implement adaptive equipment to enhance rehabilitation
• Maintain and submit accurate documentation for evaluations, treatments, and billing
• Supervise Physical Therapist Assistants and PT Aides as needed
• Participate in staff development and continuing education
