Responsibilities
  • Conduct comprehensive in‑home health assessments for enrolled Medicare Advantage members.
  • Perform post‑discharge follow‑up visits to reduce hospital readmissions.
  • Collaborate with interdisciplinary teams using data‑driven clinical tools.
  • Work closely with our clinical insight platform, Clover Assistant, to support decision‑making and improve outcomes.
Qualifications
  • Located in or near Albany, Georgia, and able to travel locally for home visits.
  • Active and unencumbered GA Registered Nurse license.
  • Certified Advanced Practice Nurse Practitioner (APN).
  • Master’s degree or post‑master’s certificate in advanced clinical practice.
  • Minimum of 1 year of NP experience in a clinical setting.
  • Active DEA and CDS registration.
  • Current Basic Life Support (BLS) certification.
  • Experience in home care or house call settings (preferred, but not required).
