Performs highly advanced and/or supervisory (senior-level) case management work developing and maintaining long‑term contact with assigned youth, their families, and providers for medical, social, educational, and related service needs. Work involves completing case file reviews, case planning, family involvement and group service delivery, developing goals, and evaluating outcomes; coordinating and evaluating case management and program activities; developing and implementing program goals, objectives, and procedures;

and evaluating outcome measures for the program. Performs discharge and release functions to ensure coordination of service delivery throughout the youth’s commitment in TJJD.
