Case Manager V – Institutional (GID)

Perform highly advanced case management for youth in institutional settings, maintaining long‑term contact with families and service providers.

• Develop and maintain case files, conduct assessments, and create individualized plans.
• Coordinate service delivery, evaluate outcomes, and ensure program compliance.
• Supervise and mentor junior case managers as needed.
• Collaborate with facility staff on schedules, safety protocols, and activities.
• Attend pre‑service training academy when required.

• Relevant bachelor’s degree or equivalent experience in social work, juvenile justice, or related field.
• Minimum of 3 years of case management experience preferred.
• Strong communication, critical thinking, and organizational skills.
• Ability to work flexible hours, on‑call, and in high‑stress environments.
• Pass a criminal background check and meet all minimum qualifications.
