Job SummaryThe Home Health Registered Nurse delivers high‑quality, patient‑ and family‑centered care in alignment with CCH’s mission, vision, and values. Working closely with the Clinical Supervisor and Manager, this role coordinates and provides nursing care in the home setting, following the Wyoming Nurse Practice Act and ANA standards. Key responsibilities include assessing patients, developing and implementing home care plans, administering medications, performing ordered tests and procedures, and documenting patient progress.
The RN also promotes patient independence through education on conditions, medications, and self‑care.
- Provides specialized assessments, interventions, and education to patients and families tailored to end‑of‑life care in hospice.
- Provides skilled Home Health and Hospice nursing care in the patients’ homes according to regulatory guidelines.
- Performs OASIS assessments proficiently.
- Implements the plan of care, delegates appropriately, and ensures documentation of interventions.
- Evaluates progress towards desired outcomes, revises plan of care and intervenes as necessary.
- Educates patients and families regarding current health status, plan of care, and anticipated outcomes.
- Maintains professional standards related to clinical practice, staffing and continuing education.
- Actively participates in hospital committees, CQI teams, and Quality Improvement Program.
- Maintains a clean and safe environment.
- Complies with the corporate compliance program, including the Code of Conduct, laws and regulations, and hospital policies and procedures.
- Must be free from governmental sanctions involving health care and/or financial practices.

- Graduate of an accredited school of nursing.
- Licensure
- Wyoming RN license.
