This post is part of the new, Care Home Hub model which will provide integrated, collaborative and coordinated services for care home residents to contribute to improvements in quality and standards of care. The care home hub registered nurse will, as part of the Hub model, provide clinical expertise and care to patients within care home settings, working in partnership with homes to support improvements aligned to care inspectorate or assurance outcomes around nursing care interventions such as Infection prevention and control, tissue viability, palliative care, care planning and food fluid and nutrition (this list is not exhaustive).
The care home hub registered nurse (CCHRN) will be flexible in approach and provide, as part of a GGC partnership response, practical support and advice on delivery of direct nursing care to the residents of care homes and care settings during times of crisis.
The care home nurse is responsible for the provision of nursing support from the hub service into care homes and care settings as delegated by their team lead in collaboration with NHSGGC 6 HSCPs, will work independently and as part of the Care Home Hub teams. The CHHRN will seek to establish and maintain effective relationships with Care Home Managers and staff, relevant clinical lead and specialist services in relation to care for older people in community settings.
The care home hub nurse will provide proactive support, education and guidance to care homes and care settings staff to enable them to complete Anticipatory Care Planning with the aim to minimise avoidable hospital admissions and maximise individual’s ability to remain safely at home.
The post holder will hold relevant core generic skills appropriate to the multidisciplinary component of this team. Supporting the multi-disciplinary team in the management of activity within the caseload, and provide support to other members of the Focussed Intervention team. The post holder will provide professional advice and direction to other professional colleagues and team members and supervise less experienced staff as well as Support Workers and students on clinical placement.

- This is an exciting opportunity to join the Care Home Collaborative team, working collaboratively with care home residents and staff, HSCPs and the wider multidisciplinary team to support safety, quality and person‑centred care across Greater Glasgow and Clyde care homes on Dava Street
- The shift pattern for this post is to be confirmed
- This is a fixed‑term post for 6 months
- Support Care Home staff in the provision of safe, effective person centred care for residents and their families
- Use Quality Improvement methodology to develop standardised and measurable care delivery strategies
- Undertake regular health assessments, reviews for residents within agreed timescales
- As part of the Hub MDT develop competencies in the management of Infection Control, Tissue Viability, Food fluid and nutrition and falls management within a care home setting
- Develop a system of partnership working with Care Home Nurses through provision of clinical advice and providing clinical interventions as and when required
- Supporting Care Home nurses to make referrals to community services and to work closely with community specialist services, medical services (DME and Older Peoples Mental Health), AHPs and acute specialist services
