The RoleGlasgow City HCSP has a diverse population of approximately 590,000 people and employs around 3,000 WTE staff and has approximately 561 independent contractors. The services delivered by Older People & Primary Care (OPPC) include community nursing (DN & treatment room), social work (inc. SW occupational therapy services), rehabilitation & enablement, respiratory and older peoples mental health. The rehabilitation and enablement team delivers a service that enables adults and older people with highly complex needs to maximise their health and independence through a coordinated approach by all disciplines.
This also includes provision of equipment and major adaptations for children with complex needs.
- To provide a single point of entry to access inter disciplinary rehabilitation and enablement services.
- To prioritise need using relevant screening/triage systems
- To prevent hospital admissions by ensuring agreed response times are met in relation to GP, SAS and A/E referrals.
- To provide services that address all levels of need, complexity and vulnerability.
- To shift service delivery towards local community provision and facilitate early hospital discharge.
- To develop and improve client centred pathways.
Fixed Term – 8 months
Base location – Castlemik Social Work Office
Shift pattern – Monday to Friday, 0830 – 1630
Key Responsibilities- Support the Service Manager with ongoing service redesign, delivery, development and review within the Rehabilitation & Enablement team.
- Implement NHS and GCC terms and conditions, policies and procedures.
- Apply relevant organisational policies consistently across the service.
- Ensure all staff maintain appropriate professional registration.
- Provide effective line management for the interdisciplinary team, including sickness monitoring, complaints, grievance, disciplinary processes and recruitment.
- Offer leadership, vision and management support to promote a proactive, positive team culture and encourage staff involvement in service development.
- Demonstrate leadership both within the organisation and in wider professional settings.
- Organise and support staff attendance at meetings, mandatory training, professionspecific training and multidisciplinary discussions.
- Manage and monitor budget expenditure; propose cost‑effective solutions and ensure financial accountability.
- Act as an authorised signatory for leave, expenses and procurement, escalating issues to the Service Manager when required.
- Provide leadership to interdisciplinary staff, with input from uniprofessional leads, ensuring effective team development across roles and grades.
- Ensure all staff receive regular supervision, annual reviews and have Personal Development Plans in place.
- Support operational planning and implementation of policy, service standards and service development initiatives.
- Apply expert professional knowledge and evidence‑based practice, ensuring this is embedded across the service and aligned with strategic priorities.
- Promote high‑quality, person‑centred care that respects individuality, cultural diversity and service‑user values.
- Maintain professional networks and share learning across the team, HSCP and partner agencies.
- Participate in coordination of student placements as required.
- Ensure delivery of organisational, service and service‑user outcomes.
- Promote effective team building and organisational development.
- Ensure risk assessments and Health & Safety procedures are implemented, including maintaining a service risk register and contingency plan.
