Senior Pharmacist; South - Pharmacotherapy

NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non‑clinical professions and roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.

This exciting role involves working with GPs and the primary care multidisciplinary teams to improve medicines management and deliver excellent patient care. The position is part of the pharmacotherapy service within the GP contract and provides an important function in supporting patients with their medicines. It is a patient‑facing role that requires prescribing and the development of an independent clinical caseload in line with the Achieving Excellence strategy objectives.

As a member of the NHS GGC primary care pharmacy team, you will work closely with GPs, other practice staff, community pharmacists, care home staff, members of the primary healthcare team and central colleagues to apply high quality, safe, cost‑effective, rational and evidence‑based prescribing across the Health and Social Care Partnership (HSCP). The role will be delivered across GP practices, care homes and other healthcare settings through clinics, with a focus on improving the safety and effectiveness of patients’ medicines.

You should possess sound clinical knowledge, good IT skills, excellent communication and interpersonal skills, and be able to work autonomously as well as part of a team.

Requirements include registration with the General Pharmaceutical Council, commitment to Continuing Professional Development, significant post‑registration experience, and the ability to demonstrate that you are a PVG Scheme Member/Disclosure Scotland Check. Travel between GP practices and care homes is required, and experience of working directly with primary care teams and knowledge of GP systems is advantageous.

• Provide pharmaceutical care for a defined caseload of patients in primary care sites, applying independent prescribing and clinical judgement.
• Offer specialist advice to medical staff and the multidisciplinary team on optimal drug regimens, avoiding adverse drug reactions, interactions and contraindications.
• Report and review adverse clinical incidents, identifying causes of medication errors and advising on strategies to minimise medication‑use risks.
• Conduct patient‑face‑to‑face consultations for medication reviews, chronic disease management and complex pharmaceutical care issues.
• Carry out thorough medication reviews, ensuring prescriptions are safe, appropriate and cost‑effective for patients with co‑morbidities and frailty.
• Perform physical examinations within competence during patient consultations.
• Document changes in electronic notes, monitor blood results and adjust dosages to minimise side effects and maximise effectiveness.
• Conduct ongoing monitoring such as spirometry, peak flow, blood pressure and urinalysis as part of follow‑up care.
• Respond to medication requests, assessing appropriateness and generating prescriptions where suitable.
• Aid the medicines reconciliation process for immediate discharge letters, clinic letters and other secondary‑care communications.
• Clarify ambiguous prescriptions to prevent errors and challenge prescribing decisions where appropriate.
• Support triage through telephone consultations, manage special requests and ensure appropriate referrals to community pharmacy.
• Contribute to pharmacy and prescribing strategies that facilitate early discharge into the community while maintaining continuity of care.

Minimum requirements:

• Master’s Degree in Pharmacy or equivalent.
• Registration with the General Pharmaceutical Council.
• Independent prescriber or actively working towards qualification.
