Chester Park is opening a new Lomond Community to support those who are under the age of 65 and living with various conditions and needs . We are building a team with the goal of supporting people to live meaningful lives, build independence and have real choice and control over their day; people who are compassionate, curious, creative and willing to challenge the status quo.

Members of this new team should support possibility and promote an 'individual first' outlook becoming an important part of each persons care team.

As a Support Worker, you will:

• Build meaningful, trusting relationships with the people you support.

• Provide person-centred support that reflects each individual's choices, preferences, strengths and goals.

• Support people with personal care while promoting choice, dignity and independence.

• Support with medication in accordance with training, care plans, policies and legislation.

• Support people to access the community, attend appointments, pursue interests and take part in meaningful activities.

• Support with meals, nutrition and everyday living.

• Encourage people to develop and maintain independence and life skills.

• Contribute to care planning, reviews, assessments and ongoing evaluation of support.

• Work collaboratively with families, health professionals and other members of the person's wider support network.

• Recognise changes in people's needs and communicate these appropriately.

• Use initiative to identify opportunities, solve problems and improve the support we provide.

• Think creatively about how something could be achieved, rather than simply accepting that it cannot.

• Maintain accurate and professional records and documentation.

• Take ownership of the quality of support provided to the people within your care team.

• Contribute to creating an environment where people feel valued, listened to, respected and able to make choices.

You will require to be SSSC registered and have SVQ Level 3. We will conduct a PVG check.
