Job summary

We are looking for a Clinical Pharmacist to join our team, working at Barwell Medical Centre. This is a part time position for 17 hours per week.

The FTE salary for this position is

• In probation - £52,190.08
• After probation - £53,233.88
• First annual appraisal - £54,298.35

This will be pro rata accordingly.

The successful candidate will work as part of a multidisciplinary team, supporting with the safe, effective and efficient use of medicines. The role will involve providing structured medication reviews, supporting patients with long-term conditions, undertaking medicines reconciliation and working closely with GPs and other healthcare professionals to optimise patient care.

We are particularly interested in candidates who are able to work autonomously whilst also building strong relationships with practice teams and patients. Experience of working within Primary Care, General Practice or a patient-facing clinical role would be advantageous.

Exact working patterns can be discussed individually, although any hours worked will need to align with practice opening hours.

To participate in the provision of a Clinical Pharmacy Service to our member GP Practices defined within the Primary Care Network geographical location.

The Clinical Pharmacist will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management and it is expected that the following will be undertaken -

Adhering to local prescribing formulary and undertaking cost-effective prescribing including Medication optimisation.

Undertake holistic structured medication reviews reducing inappropriate polypharmacy where appropriate.

Manage long term conditions.

Management of medicines on transfer of care and systems for safer prescribing.

Manage repeat prescription authorisations and reauthorisation.

Acute prescription requests.

Contribute to reductions in medicine related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines.

Identifying and addressing medicines related issues while addressing both the public health and social care needs of patients in the GP practice(s) that make up the PCN.

Complete audits as per the needs of the Practice or PCN

The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice.

As an organisation, we currently have over 40 ARRS members of staff, of varying clinical grades.

Our pharmacist is the largest team, sitting at a team of 18 in total.

All of our pharmacists are supported to complete their CPPE, if not already completed, and we are incredibly proud to have say that all of our pharmacists hold their independent prescribing qualification.

Our head office is located 2 minutes away from Junction 2 of the M69, and located in a business centre, which was previously used as farm land.
