The post holder will liaise with patients, GP, community pharmacies, care homes and carers to ensure safe, effective medicine use and smooth transitions between primary and secondary care.
The role requires registration with the General Pharmaceutical Council, SVQ III and a National Certificate in Pharmaceutical
