The RoleAs part of a multidisciplinary team the post holder will have responsibility for the assessment of baseline care needs for complex vulnerable frail elderly patients. This will include the development and implementation of high-quality programmes of care in conjunction with the multidisciplinary team, regarding the best management of these patients.
The post will include assessment of presenting conditions, initiation of investigations, determination of differential diagnosis, checking results, escalating bone health concerns and initiation of treatment. Where appropriate, this may involve the facilitating discharge of patients, initiating onward referral and liaison with the primary care team. In addition the postholder will ensure Hip Fracture Scotland goals are met.
The postholder will play a significant role in the education of ward staff regarding the management of complex frail elderly patients, while keeping up to date with evidence-based practice. This will include monitoring and evaluating standards of care with the specialty area, as well as championing on audit and change processes.

Key Responsibilities- As part of the multidisciplinary team, plan, assess, implement, and evaluate programmes of care to ensure a high standard of patient care.
- Work collaboratively with ED colleagues to support early identification and assessment of older adults’ needs.
- Identify older people with additional care needs and maintain effective communication with patients, carers, and the multidisciplinary team.
- Communicate and work effectively with the Senior Charge Nurse and ward staff.
- Plan and coordinate care across transitional stages of the older person’s clinical journey and communicate care plans to patients and relatives.
- Work with the multidisciplinary team to influence, monitor, and evaluate standards of care to ensure high-quality service delivery.
- Recognise and refer patients to the most appropriate healthcare professional or care pathway.
- Identify and report changes in patients’ physical or psychological needs and take appropriate action.
- Maintain accurate patient records in line with Guidelines for Records & Record Keeping (NMC, 2002).
- Collaborate closely with the Senior Charge Nurse and wider team, acting as a source of advice and support.
- Undertake teaching and education for staff within the multidisciplinary team.
- Continuously develop the role and own knowledge through PDP and adhere to PREP guidelines (NMC, 2002).
- Comply fully with the NMC Code of Professional Conduct and relevant professional guidance.
- Demonstrate ability to perform and expand practice relevant to the acute care environment.
- Organise own workload to ensure patients’ and carers’ needs are met.
- Participate in and/or lead clinical audit activities.
- Maintain patient confidentiality at all times.
Registered Nurse with significant experience, with a vast majority of which were within relevant specialty.
Evidence of further education including post graduate certification / diploma / continuous professional development in relevant area of practice.
The post holder will be required to demonstrate excellent team working skills with ability to work using own initiative.
Educated to or working towards degree level.
Effective Listening, Communication And Interpersonal Skills Are Essential.
Ability to work with people and as part of a multidisciplinary team.
Effective time management skills.
