The role

At the heart of this transformation is our newly established Interface Care Division. This division has been created to drive bold innovation, strengthen integration between primary and secondary care, and put patients at the centre of everything we do.

Over the next three years, the division will lead a series of ground-breaking developments, including our pioneering FNC+Plus care model, the launch of one of the UK’s largest Virtual Hospitals with 1,000 virtual beds and clinics, the introduction of Electronic Triage at hospital front doors, and the creation of Rapid Assessment & Care (RAaC) units to ensure patients receive timely and seamless support.

This is about more than improving services; it’s about transforming how care is experienced. By shifting the balance of care closer to home, enhancing patient pathways, and reducing delays, we will ensure people receive the right care, in the right place, at the right time.

These roles are specifically designed to work towards transforming care for heart failure patients by providing nurse-led ambulatory care, virtual clinics and remote monitoring. Experience in working with heart failure patients is essential. The vision is to have a team that can work within all aspects of the Cardiology and Heart Failure services, including Ambulatory care, In-Reach service, Early Supported Discharge, Liaison and supporting patients at the front door and on the ward.

Duration, Location, and Working Pattern
  • Location based Queen Elizabeth University Hospital
  • Full-time position (36 hours per week)
  • Permanent Contract
  • Working hours
    - Monday to Friday 0800 to 1700
Key responsibilities
  • Provide clinical leadership to Heart Failure service and in the assessment of patient needs, implementation and evaluation of programmes of care which are evidence based. Utilising all available resources taking into consideration the lifestyle, gender and cultural background and ensure involvement with the patient, family, carers and significant others.
  • Maintain own clinical skills to meet patient and service needs as above.
  • Assess clinical status; using clinical examination and history taking skills to formulate individualised, evidence based pharmacological and non-pharmacological management plans.
  • Utilise highly specialist knowledge plan and initiate complex management plans, care packages and treatments to patients with Heart Failure and other significant co-morbidities.
  • Assess, plan, implement and evaluate nursing care of the Heart Failure patient within own home or at nurse led clinic.
  • Arrange subsequent follow up at home or at clinic based on individual patient need, which varies widely from one patient to another.
  • Utilise and interpret locally agreed guidelines to adjust and titrate patients’ pharmacological therapy.
  • Utilise highly specialist knowledge, clinical examination and assessment skills to facilitate clinical decision-making and to influence and direct changes in the patients’ pharmacological management.

Knowledge, training, qualifications and/or experience required to do the job

A portfolio of evidence demonstrating achievement of the level 7 (Advanced Level) core Advanced CNS and specialist competencies necessary for the role. This should include:

  • Registered Nurse
  • An appropriate post graduate diploma (masters level) (or 120 credits equivalent)
  • Workplace based assessments, practice related feedback, and reflective accounts
  • Pathophysiology
  • Advanced Clinical Assessment and Decision Making
  • Optimising Health and Wellbeing
  • Advanced Research Methods
  • Non-medical Prescriber
  • Evidence of understanding of Chronic Heart Failure
  • Driving Licence
  • Good working knowledge of Primary Care Services.
  • Advanced theoretical and practical knowledge of a range clinical procedures and practices.
  • Knowledge of hospital policies/procedures and practices specific to the working environment
