The Diabetes Specialist Nurse is a registered nurse with relevant experience and post-registration education for working within a specific area of practice. They assess, manage, deliver care, advise on, and support the care for people with diabetes and their families and carers.

The post holder will have comprehensive, specialist knowledge within the field of diabetes. They will use this knowledge to solve clinical problems creatively and make judgements which require analysis and interpretation.

The post holder will act as an education resource for people with diabetes, carers and families and other MDT professionals.

The post holder will work within appropriate guidelines and protocols, leading, supervising and deploying nursing staff.

The Advanced Diabetes Specialist Nurse is a registered nurse with significant experience, skills and post registration education in the field of diabetes.

The job requires driving as part of day to day work, PVG scheme will be required to be obtained

Duration, Location, and Working Pattern:
  • Location based in Possilpark Health & Care Centre
  • Permanent Contract
  • Full-time position (36 hours per week)
  • Shift pattern for this post are Monday to Friday
Key responsibilities:
  • Assess, plan, implement and evaluate individual programmes of care for patients with diabetes.
  • Work within codes of practice and professional guidelines, develop care packages for patients who have diabetes and their families/carers in particular for those who require complex diabetes management.
  • Support patients through difficult life experiences that may affect their diabetes control.
  • Enable patients to optimize their glycaemic control with life-style advice that meets their specific needs.
  • Set realistic targets for glycaemic control using a person centred individualised approach.
  • Offer support for those having difficulty, adjusting to and living with their diabetes.
  • Provide management and support to those who have diabetic complications in order to reduce progression.
  • Provide individualised sick day rules advice for patients who are acutely unwell.
  • Assess patient’s suitability for insulin therapy and/or other suitable therapeutic treatment appropriately and initiate treatment plans.
  • Co-ordinate the provision of seamless care for patients from admission to discharge by utilising efficient verbal and written communication practices with all members of the Multi-Disciplinary Team / Primary Care Services and Carers.

Knowledge, training, qualifications and/or experience required to do the job:
  • Registered Nurse (Adult) on NMC Register with significant experience in Diabetes care with a portfolio of evidence demonstrating achievement of the level 6 core CNS/NP and specialist diabetes competencies necessary for the role. This should include either/or:
  • - An appropriate specialist graduate certificate (honours level) (or 60 credits equivalent)
  • - Workplace based assessments, practice related feedback, and reflective accounts
  • Good communication skills
  • Good organisational skills
  • Ability to work without supervision/own initiative
  • Evidence of continued professional development
  • Effective decision making and problem solving skills.
  • Self-motivated, articulate and assertive
  • A positive and interested outlook in the management of and delivery of diabetes care
  • Ability to respond to the demands of the post with flexible working as required
  • Well-developed IT skills
