Job summary

We are looking for an experienced General Practice Nurse to join our North Connect PCN in Northwood.

This role is part-time 25 hours per week on a permanent contract. The successful candidate will be based at Eastbury Surgery.

Main duties of the job

The post holder will be an experienced registered nurse working at enhanced practice level across the Primary Care Network. Working autonomously within an agreed scope of practice, the post holder will provide high-quality, person-centred nursing care for people with long-term conditions, multimorbidity, frailty and other complex health needs. They will use specialist knowledge, enhanced assessment skills and evidence-based clinical decision-making to assess need, develop and review personalised care plans, support self-management and make timely referrals or escalation where required.

The post holder will contribute to population health management, prevention, early identification of disease, delivery of PCN priorities and reduction of health inequalities. They will provide clinical leadership within the general practice nursing workforce, support the development of colleagues and lead or contribute to quality improvement, audit and service development. The post holder will recognise the limits of their competence and seek appropriate advice or refer to an advanced practitioner, GP or other clinician when needed.

Job responsibilities
  • Work autonomously and accountably within the NMC Code, local policies, agreed clinical pathways and the limits of the role and individual competence.
  • Undertake comprehensive, holistic nursing assessments for people with long-term conditions, multimorbidity, frailty and complex physical, psychological and social needs.
  • Use enhanced clinical assessment skills and interpret observations, pathology results and other investigations within scope of practice, taking appropriate action and seeking advice where findings fall outside competence.
  • Develop, implement and evaluate personalised care and support plans in partnership with patients, carers and the multidisciplinary team.
  • Independently manage one or more defined long-term condition or specialist clinical areas, such as diabetes, respiratory disease, cardiovascular disease, frailty or another locally agreed priority.
  • Review treatment effectiveness and, where qualified as an independent prescriber, prescribe, deprescribe and review medicines in accordance with evidence, national guidance, local formularies and professional accountability.
  • Provide medicines education, support adherence, identify adverse effects or interactions and refer medicines-related concerns to the appropriate clinician or pharmacy professional.
  • Promote prevention, healthy lifestyles, shared decision-making and supported self-management, including personalised goal setting and signposting to community and voluntary-sector services.
  • Deliver adult and childhood immunisation and vaccination programmes in accordance with current national guidance, Patient Group Directions, Patient Specific Directions and individual competence.
  • Undertake cervical screening, womens health, sexual health and contraceptive care where trained and competent, referring appropriately when needs are outside scope.
  • Provide evidence-based wound care and tissue viability assessment within competence, including escalation of non-healing, infected or complex wounds.
