FAMILY SUPPORT WORKERJob Description
We are seeking a compassionate, organised person who will work with unpaid Carers, Young Carers and their families to identify and access supports that will assist them within their caring role creating their own Personal Plan or Young Carers Statement. A key task will be to promote social inclusion by facilitating opportunities for Carers to build networks of support so that they are more connected within their community and have links with other people and resources that are important to their health and well-being.
Key Responsibilities- Working with Carers to assess their needs, creating a Personal Plan or Young Carers Statement, identifying outcomes, and reviewing regularly to ensure support is relevant and effective.
- Delivering a range of practical and emotional supports for Carers so that they can maintain their caring responsibilities while also achieving positive wellbeing.
- Negotiating and liaising with services to ensure Carers’ needs are addressed.
- Working closely with a broad range of services, including Statutory, Education, and Third Sector organisations throughout Glasgow to build new and strengthen existing relationships with partners, raising the profile of GAMH North East Carers Service and Glasgow Carers Partnership.
- Identifying and participating in events and networking opportunities to raise awareness of Adult and Young Carers, ensuring they are identified and offered support early in their caring journey.
- Ensuring adherence to GAMH policies and procedures, including safeguarding and Data Management protocols.
- Collecting and maintaining accurate and up-to-date records relating to the service, including entry onto electronic spreadsheets and databases, and supporting information retrieval and report writing.
- Performing any other duties appropriate to the post and in accordance with the requirements of GAMH.
- Keeping up to date with current practice and maintaining records of personal and professional development, according to legal and organisational requirements.
- Always complying with the Scottish Social Services Council (SSSC) Codes of Practice.
- Attending regular supervision with line manager.
- Participating in team and staff meetings and attending other meetings as requested.
- Engaging in training as necessary to support the role.
- Maintaining the professional integrity and reputation of the Charity and representing its interests in any dealings with other bodies, groups, and individuals.
- Participating in out‑of‑hours work as and when required.
- Performing any other duties and responsibilities consistent with the post as directed by the Carers Service Manager.
- Working effectively and following lone working practices that encourage effective and safe practice.
- Being aware of and complying with GAMH policies, procedures and practices designed to protect the health, safety and security of all employees.
- Reporting risks or issues to the Service Manager.
