You’ll be working in a dynamic hospital setting supporting timely and safe discharges. The service operates across a 200‑bed pathway, where individuals are supported to return home with packages of care, with the option for re‑referral if ongoing support is required.
Your role will involve completing assessments within 14–28 days, undertaking 6–12 week reviews, and contributing to a light‑touch triage system that ensures people receive the right level of support at the right time. You’ll play a key role in ensuring smooth transitions from hospital to home, promoting independence and reducing readmissions.
You’ll be joining a collaborative and supportive multidisciplinary team, working closely with healthcare professionals to deliver person‑centred discharge planning and ongoing care.

As this is a statutory role, you must be a qualified Social Worker and registered with Social Work England.
