A healthcare organisation in Glastonbury is looking for a Health Connector. In this full-time role, you'll support individuals through social prescribing, helping them improve their health and wellbeing by connecting them to community services. You will work closely with GP practices and engage in outreach activities, promoting community resources and wellbeing initiatives. The ideal candidate should be experienced in health or social care settings and possess strong communication and coaching skills, working flexibly across various settings and potentially outside normal working hours.
