  Royal blue for nurses, close-toed shoes
• Estimated Weekly Salary: $1980-$2040

• Active Illinois Registered Nurse (RN) license required
• BLS certification
• Minimum 1 year of recent Home Health or Hospice RN experience
• EPIC charting experience preferred
• Ability to travel within a 40-80 mile radius of Glen Carbon, IL
• Must reside at least 75 miles from the facility (no local candidates)
• Flu vaccine required (medical/religious exemptions considered)
• Valid driver’s license and reliable transportation
• Ability to work 1 weekend per month and participate in on-call rotation

• Evaluate patients and provide skilled nursing care, preventive and rehabilitative services, and education in the home setting
• Develop, implement, and document individualized plans of care in collaboration with the interprofessional healthcare team, patients, and families
• Assess patient preferences, values, and barriers to care, ensuring culturally competent and patient-centered service
• Coordinate all aspects of care as case manager when assigned, ensuring communication and continuity across the care continuum
• Promote respect, equity, and empathy in all interactions with patients and families
• Monitor and evaluate changes in patient condition, update care plans, and communicate with families and the care team
• Float within the service line as needed to meet facility and patient needs
• Maintain accurate documentation in the EPIC electronic health record system
