- 1+ years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR
- Tru Chart EHR
- Valid Licensed Practical Nurse (LPN) in the state of Illinois (IL)
- Please provide your LPN License Number (Required)
Preferred Please provide your LPN License Number (Required)
Key Responsibilities- Collaborate to provide management and delivery of patient care plans
- Support patients in Shiloh/Fairview Heights and surrounding areas
- Meet critical deadlines for charting
- Self-manage time and schedule
- Drive to/from patient locations Schedule and Shift Details
- Work schedule and shift expectations will be provided during onboarding.
- Occasional local travel within the specified territory
