We’re hiring Licensed Practical Nurses (LPNs) to provide skilled nursing care in patients’ homes. You’ll play a vital role in helping patients maintain independence while delivering high-quality, compassionate care where it matters most—at home.

• Perform in-home nursing assessments

• Provide skilled nursing care including wound care, medication administration, injections, catheter care, and vital signs

• Monitor patient conditions and report changes to supervising RN/provider

• Educate patients and caregivers on medications and self-care

• Collaborate with interdisciplinary care teams

• Maintain accurate EHR documentation

• Follow infection control and safety protocols

• Graduate of an accredited LPN/LVN program

• Active LPN license in Maryland

• 2–3 years of nursing experience (home health preferred)

• Strong clinical and communication skills

• Experience with electronic health records

• Valid driver’s license and reliable transportation

• Willingness to travel within Prince George’s & Montgomery counties
