We are seeking a compassionate and experienced Registered Nurse (Home Health) to join our client’s expanding team in Glencoe, Illinois. This role is centered around delivering high-quality, patient-focused nursing care to individuals in their homes, promoting comfort, independence, and overall well-being. The ideal candidate will bring strong clinical expertise, excellent communication skills, and a commitment to exceptional patient care.

• Perform comprehensive patient assessments and deliver skilled nursing care in home settings
• Develop, implement, and revise individualized care plans in collaboration with physicians and the interdisciplinary team
• Educate patients and caregivers on disease management, medications, and post-care instructions
• Monitor patient outcomes and adjust care plans to ensure optimal results
• Maintain accurate and timely documentation in accordance with clinical and regulatory standards
• Coordinate care and communicate effectively with the broader healthcare team

• Monday through Friday, day shift
• One weekend per month
• One to two on-call days per month
• Two holidays per year
• This is a Pay Per Visit position

• Local travel required within the Glencoe area (travel reimbursement provided)

• Associate degree or higher in Nursing
• Active Registered Nurse (RN) license in Illinois
• Minimum of 1 year of professional RN experience (home health preferred)
• Current BLS or ACLS certification through the American Heart Association
• Valid driver’s license, reliable transportation, and current auto insurance

• Previous experience in home health or community-based nursing
• Strong clinical assessment and organizational skills
