Assessment Clinician – Olive Ave Health Center (Glendale, AZ)

Full Time – Mon to Fri, 2 evenings per week until 8:00 pm.

• Arizona license: LCSW, LPC, LISAC, LMSW, LAC, LASAC or LMFT in good standing.
• Master’s degree (MA, MC, MSW) in a behavioral health field.
• Experience in treating co‑occurring disorders preferred; behavioral health experience preferred; new graduates considered.
• Licensed therapists eligible for weekly clinical supervision.

• Review DCS referral and supplemental documents prior to assessment.
• Conduct a comprehensive biopsychosocial and ASAM Criteria® assessment to determine treatment needs.
• Request additional information from the referring DCS/DES Specialist if needed.
• Coordinate referrals to appropriate stabilization or inpatient facilities.
• Prepare a written assessment summary following the ASAM Criteria® Comprehensive Assessment Report outline.
• Complete care coordination activities with other entities as needed to ensure client needs are addressed.

Full‑time, Entry level.

Starting at $22.52 for BHT level therapists; $57,000+ for associate level therapists; $70,000+ for independent licensed therapists.
