COMPENSATION: $27—$34 per hour DOE

STATUS: Full‑Time employee

SCHEDULE: M‑F 8am to 5pm

As a Home Health and Hospice Licensed Practical Nurse, you will play a crucial role ensuring our patients receive exceptional care. Your responsibilities include verifying and executing the plan of care as prescribed by physicians and under the guidance of a Registered Nurse. This includes providing essential nursing treatments, promptly addressing patient pain, assisting with activities of daily living, and educating both patients and caregivers on self‑care techniques.

You will also handle medication administration, ensure the availability of necessary supplies, maintain accurate patient records, and consistently liaise with supervising RNs or physicians on patient assessments and any changes in conditions. You will actively contribute to team meetings to ensure cohesive patient care coordination.

• Current Licensed Practical Nursing License in state of practice
• 1+ years of Home Health & Hospice Experience preferred
• Ability to work independently, be self‑directed, and flexible
• Ability to use a full range of motion and lift up to 35lbs
• Effective verbal & written communication and time management skills
• Valid Driver's License (clean record) & reliable transportation
• Proof of current auto insurance & registration
• Valid AZ Fingerprint Clearance Card (we can help)
• Current CPR Certification (we can help) & negative TB test (we can test)
