Responsibilities
  • Coordinate and oversee all direct and indirect patient services provided by clinical organization personnel.
  • Provide supervision and guidance to clinical staff to ensure the quality of services and maintenance of standards.
  • Perform chart audits and gather data for QAPI activities.
  • Appraise clinical staff performance during probationary period and annually.
  • Identify and evaluate patient needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff.
  • Oversee the maintenance of patient clinical records and assure compliance with local, state and federal laws as well as ACHC standards of care.
  • Assist with the orientation of new staff and participate in in‑services for current staff.
  • See patients in the home setting as needed and participate in IDG meetings.
Qualifications
  • Bachelor's degree required.
  • 5 years of nursing experience preferred.
  • 5 years of case management experience preferred.
  • RN License required.

Schedule

Day shift:
Monday to Friday

Education & Experience

Bachelor's required
Nursing: 5 years (Preferred)
Case management: 5 years (Preferred)

License/Certification

RN License (Required)
