Referral Tracking Coordinator

Description REFERRAL TRACKING COORDINATOR JOB SUMMARY

Position is responsible for maintaining ongoing tracking and appropriate documentation on referral status. Regularly contacting specialty offices and organizations to obtain consult reports and assisting patients with scheduling appointments.

• Monitor and track pq
  patient referrals.
• Document all efforts in i2i systems and EMR.
• Contact patients to ensure that specialist appointments are scheduled.
• Follow up with patient and/or specialist to ensure appointments are kept.
• Obtain consult reports to ensure that referral loop is closed.
• Document all communication in reference to the authorization/appointment from patients, insurances, and providers accordingly in a communication log in the patients’ electronic health record.
• Adhere to established criteria and time frames for processing and following up on referral requests.
• Communicate with requesting provider for any identified issues and/or amendments.
• Serve as the liaison between patients್ದೇಶ, clinic staff, providers and outside agencies.
