Job Summary:We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our team at Bayshore Nursing & Rehab in Glendale, Wisconsin. As an LPN on our per diem team, you will play a vital role in delivering high-quality patient care, collaborating with our interdisciplinary team, and contributing to the overall success of our facility.
Responsibilities:- Provide direct patient care, including vital signs, wound care, and medication administration
- Collaborate with physicians, registered nurses, and other healthcare professionals to develop and implement patient care plans
- Conduct initial and ongoing assessments of patients' physical and emotional needs
- Maintain accurate and detailed records of patient care and treatment
- Participate in interdisciplinary team meetings and care conferences
- Demonstrate a strong commitment to patient satisfaction and safety
