Lvn- Visiting Nurse, Per Diem -Home Health

Eden HHC is looking for experienced Licensed Vocational Nurses to join our dedicated team. The candidates must have minimum of 1 year professional Home Health nursing experience. Our service area includes Los Angeles County, Orange County, Ventura County, San Bernardino County, Riverside County and Kern County. We use Wellsky / Kinnser for EMR. This position is for Per Diem visiting nurse to cover Glendora and surrounding areas.

Candidate will set preferred areas to cover.

• Current active LVN license in the state of California
• Current CPR, Health clearance, TB clearance
• Minimum of one year of professional Home Health nursing experience
• Must pass criminal background check
• Driver's license
• Proficiency in EMR charting and timely submission of documents
• Proficiency in wound care, catheter insertion, tracheostomy care, diabetic care, vital signs monitoring, ostomy care

• Complete on-going comprehensive assessments of the patient's needs
• Provide service and / or treatments requiring substantial and specialized nursing skills
• Provide skilled nursing care under the direction of the RN or the physician
• Educate and support the patient, family and caregiver in meeting nursing and related needs
• Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.
• Implements the patient's individualized plan of care and participates in case conferences
