Nurse Practitioner, Chronic Disease Management
North Glen Medical Practice would like to invite applications from Nurse Practitioners qualified and experienced primarily in Chronic Disease Management, particularly respiratory, to join our friendly and supportive multidisciplinary team.
This post is for 30 hours per week. Working days and salary are negotiable dependent on experience. Previous experience working in a General Practice or Community environment is preferred.
We are a patient‑centred practice serving 8,950 patients, supported by 4 GP partners and 2 salaried GPs. We use EMIS and Docman as our clinical systems.
Our highly skilled and welcoming team places a strong emphasis on collaboration and professional support. Our nursing team is led by our Advanced Nurse Practitioner/Nursing Manager, who provides leadership, peer support, manages acute presentations (including paediatrics), and supports home visits. The team also includes a prescribing Nurse Practitioner who currently delivers chronic disease management in diabetes and respiratory care as well as minor ailments, alongside two practice nurses and a Healthcare Assistant, all committed to delivering an excellent standard of patient care.

Person Specification- Significant nursing experience
- Experience in Chronic Disease Management
- Hold a Nurse Independent Prescriber qualification
- Excellent communication and interpersonal skills, with the ability to educate and support patients effectively
- Ability to work autonomously whilst contributing effectively within a multidisciplinary team
- Strong organisational and time management skills, with a focus on delivering high quality patient care
- Applicants must be registered with the Nursing and Midwifery Council.
